The Position:
As a Mental Health Practitioner in Mindset Horizons, you will be connected with a local and statewide team of Mental Health Practitioners, Peer Practitioners, Practice and Capability Lead and Program Leaders, as well as practitioners in our Baptcare Family and Community Services Teams.

Horizons provides short-term, psychosocial services for adults experiencing severe and episodic mental health conditions. Our Mental Health Practitioners work with people to build skills, strategies, and resources to support their mental wellbeing and work towards their goals. This role is outreach, meeting with people in their homes and communities. Horizons provides service state-wide, and this role may include travel to rural and remote areas, utilising a Baptcare fleet vehicle.

This role involves:
Engaging clients in goal and safety + wellbeing planning and supporting them to work towards these goals with a focus on capacity building.

Facilitating consumer self-assessments and incorporating them into goal and safety and wellbeing planning process.

Closure planning with a focus on ensuring clients have access to appropriate supports and resources to continue on their recovery journey when they leave the Horizons program.

Strong administration practices.

Facilitating Horizons group programs as required.

Key Selection Criteria

Ability to use a creative approach to the design of goals and skill development programs with people experiencing mental ill-health.

Excellent organisational, time management skills, and ability to be self-directed.

Effective written and verbal communication skills and able to write accurate, thorough, and comprehensive documentation.

Able to work within complex environments and be outcome-focused.

Experience using evidenced-based approaches to promote mental wellbeing.

You must also have:
Minimum Certificate IV Mental Health or equivalent

The right to work in Australia.

Australian Driver’s Licence.

Satisfactory National Police Clearance (will be completed post-interview for applicants moving onto next stage of recruitment)

Current Working with Vulnerable People Card or willingness to obtain

Current First Aid Certificate or willingness to obtain
