We are seeking an experienced and highly motivated Clinical Nurse Specialist to join our service in a pivotal Discharge Planner role. This position offers an opportunity to make a tangible difference to patient flow, continuity of care and patient outcomes by ensuring timely, safe and well coordinated discharges across the service.

Working collaboratively with a multidisciplinary team, the Discharge Planner will lead and support discharge planning processes, identify barriers to discharge early, and implement solutions that align with best practice, clinical governance requirements and organisational priorities. You will also contribute to service improvement activities, data monitoring and staff education related to discharge planning and continuity of care.

This role is ideal for a skilled clinician who thrives in a fast paced environment, communicates confidently across disciplines, and brings a calm, organised and solutions focused approach to complex care coordination.

What You Will Bring To The Role

Current registration as a Registered Nurse/Midwife Division 1 with AHPRA.Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate.Demonstrated capacity to provide effective nursing/midwifery care that is person centred, family inclusive and culturally sensitive to people accessing health services.Demonstrated working knowledge of referral pathways and services related to discharge planning and ongoing care, including COMPACS, TACP, My Aged Care, ACAT.Demonstrated effective communications written and verbal skills.Demonstrated commitment to professional development.Ability to work independently with minimal supervision maintaining involvement within the wider multidisciplinary team.Current Driver’s Licence and willingness to use for work purpose travel.
