About Miwatj

Miwatj Health is an independent Aboriginal Community-Controlled Health Service. Established in 1992, we’re committed to securing quality health care services across the East Arnhem and enabling Yolŋu powered community health and wellbeing. Miwatj provides culturally appropriate, comprehensive primary health care to over 6,000 remote Aboriginal and Torres Strait Islander residents across 3 East Arnhem wards. With a Head Office in Nhulunbuy, Miwatj also manages a Corporate Office in Darwin, 7 fixed clinics across the East Arnhem region, and dedicated outreach teams who travel to 3 additional locations.

About the Role

The Elder Care Support Coordinator role will support identified clients to navigate access to appropriate local aged care support services. They will provide advocacy support to ensure clients receive a culturally safe plan assessment.

This role includes travel to the East Arnhem region as required.

Successful applicants will have access to a range of benefits, including:

  • Competitive salary of $85,368 - $98,000 per annum + 12% superannuation
  • Ability to Salary Package up to $15,900pa plus a further $2,650pa on Meals and Entertainment

About You

You are a committed professional with experience supporting Elders, community services, or health programs, and you bring a strong commitment to Aboriginal Community Control.

You will bring:

  • An understanding of and commitment to Aboriginal Community Control and self-determination
  • An understanding of the issues faced by YolÅ‹u people living in remote settings
  • Excellent communication and interpersonal skills, with the ability to build trust with the community.
  • Demonstrated understanding of the principles of person-centred care and trauma-informed practice.
  • Understanding of how to assist and support clients to access services, reduce service complexity and confusion.
  • Good record management, data collection and reporting capability.
  • Experience in the community services field with strong community connections and networks across aged care providers, mainstream supports and community sector.
