Registered Nurse Case Manager – Full Time | Southport

Ready to take the next step in your nursing career?
If you’ve been working as a Community or Aged Care Registered Nurse for 2+ years and are looking for something fresh, read on.

We’re a rapidly growing organisation doing extraordinary things for people living with disability — and we do things differently! No agency staff. No rotating strangers.

We build consistent, skilled teams of nurses and support workers around our clients so they get true continuity of care — and our staff enjoy regular, set shifts and the chance to build real relationships.

Due to growth, we’re looking for another Clinical (RN) Case Manager to join our vibrant Head Office team in Southport (Monday–Friday, 8:00am–4:00pm).

About the Role

You’ll play a key role in managing clients (many with neurological conditions such as spinal cord injuries, MND, MS, Huntington’s, cerebral palsy, stroke, and dementia), providing clinical oversight, mentoring your in-home nursing and support teams, and ensuring the highest quality of care.

This is a mix of office and community-based work — visiting clients, onboarding new participants, conducting assessments, and training staff.

You’ll be supported by a close-knit, friendly team of about 25 at our Head Office based in Southport. Expect coffee runs, team lunches, Friday drinks, birthday celebrations — and regular visits from Winnie, our resident Frenchie who keeps everyone smiling.

Why You’ll Love It Here

·        No KPI pressure — we measure success through client satisfaction, not billable hours

·        Fun, supportive and appreciative team culture

·        Company vehicle for client visits

·        We invest in your growth — training and mentoring provided

·        Above Award pay

·        On-call allowance: $1000 per weekend (every 10–12 weeks, once you’re settled)

What You’ll Bring

  • Registered Nurse with 2–5 years’ experience

  • Case management or supervisory experience (preferred, not essential)

  • Confident communicator and strong relationship builder

  • Excellent time management and organisational skills

  • IT savvy and detail-oriented

  • AHPRA registration, NDIS Worker Screening (or willingness to obtain), Blue Card and driver’s licence

We welcome nurses from hospital, community or aged care backgrounds who are ready for a new challenge — and want to join an organisation where your skills, compassion, and professionalism are truly valued.

If this sounds like you, hit “Apply Now” — we’re reviewing applications as they come in!
