The Role
We're seeking a dedicated Care Partner to support our homecare delivery with heart and expertise. In this role you'll assist in developing client care plans, guide our care team, and ensure person-centred services that empower clients to live independently and thrive at home. Reporting to the Homecare Manager, this full-time role offers a hybrid model with flexibility to balance office-based coordination in Beaudesert and client visits across the Scenic Rim.
Key Responsibilities

Assist in client assessments, contribute to personalised care plans, and monitor support needs to promote independence and wellbeing.

Support and mentor a team of Registered Nurses, Enrolled Nurses, and Care Workers, providing day-to-day guidance and contributing to professional development.

Collaborate with clients, families, allied health professionals, and external providers to coordinate holistic care, including support for Home Care Packages.

Help ensure compliance with Aged Care Quality Standards, My Aged Care guidelines, and relevant legislation, while supporting quality improvement efforts.

Build relationships within the Beaudesert community, advocating for client rights and contributing to service enhancements.

Assist with rostering, incident reporting, and risk assessments to maintain safe, efficient homecare operations.
What We're Looking For

Relevant qualifications in Aged Care, Nursing, or Social Work (e.g., Diploma or Bachelor in Nursing, Enrolled Nurse, or equivalent); AHPRA registration desirable but not essential.

Experience (1+ years) in homecare, aged care, or community support, with a focus on client-centred outcomes.

Solid knowledge of care planning, assessments, and familiarity with Home Care Packages (training provided if needed).

Excellent communication and interpersonal skills, with a genuine passion for person-centred care and community engagement.

Proficiency in aged care software and a valid Queensland driver's licence.

Desirable: Experience in a not-for-profit or rural/regional setting.
