Aboriginal Support Coordinator – make a difference to people’s lives while working with a supportive team of mental health professionals

  • Part-time (4 days per week)
  • Service covers Darkinjung Country – Central Coast based

We have an exciting opportunity for an experienced and passionate Aboriginal Support Coordinator to join a team in our Universal Aftercare program - Care Connect, across the Central Coast region.   

Care Connect is an aftercare service supporting people experiencing a suicidal crisis or who have attempted suicide. The service focuses on empowering people and their care givers to connect with formal and practical support to assist with recovery.

At Social Futures we have a commitment to being a strong ally, nurturing relationships, walking alongside Aboriginal and Torres Strait Islander people, supporting Aboriginal leadership and self-determination and providing culturally safe services.  We are a voice for social change in regional Australia, with a history spanning over 49 years.  Your role within our dedicated suicide aftercare program, Care Connect, will allow you to:

  • Provide therapeutic supports and short-term psychological interventions, connecting Aboriginal people and families with culturally appropriate services and supports
  • Expand your case management and care coordination skills
  • Be a part of a multidisciplinary team in providing in evidence-based best practice and high quality services
  • Build partnerships with other community services and expand your network

We are looking for passionate individuals with a desire to make a difference. The successful candidates will have:

  • A Diploma or Cert IV level qualification in Mental Health or Community Services
  • Two years’ experience in a suicide prevention / aftercare or mental health setting
  • Previous experience in providing case management to vulnerable groups
  • Demonstrated experience in maintaining professional boundaries while engaging in person-centred work
