About the role:

We are looking for a highly experienced clinician to undertake comprehensive assessments of older persons to determine eligibility to access Commonwealth-funded aged care services, developing support plans and assisting in the implementation of these plans. You will provide advocacy and support to older people, families and carers across the New England North West area. This role is varied and stimulating for the range of clients encountered and the breadth of locations covered - one day you might be assessing an elderly academic in hospital, the next an Aboriginal elder or an ageing farmer at a remote location.

Working within our dynamic team, you will flexibly use different assessment modes to best meet client needs, whether this is on location with the client or remotely via virtual care. A role in this team offers flexibility that most roles just cannot consider, including options for remote working.

Where you’ll be working:

The NSW Health Aged Care Assessment service (New England) is a hugely supportive and friendly team, providing high quality assessment services to older persons in both community and hospital settings. The team covers clients resident in the New England North West – those living in the community, or in one of the 40+ aged care homes in the area, along with inpatients from 20 sites (11 hospitals and 9 MPS facilities).

Assessor base locations for the team currently are Armidale, Inverell, Narrabri and Tamworth, with other locations open to opportunity within service demands. This current opportunity is to join our team in the Tamworth area.

About you:

We are looing for someone who meets the below criteria:

  • The readiness to take on a new challenge within the freshly evolving aged care assessment space
  • Experience working within aged care (at least 2 years)
  • Excellent communication skills
  • Demonstrated ability to work in partnership with clients, families, carers and services
  • Demonstrated ability to work both independently and collaboratively within a team
  • Demonstrated ability to effectively prioritise and organise your own work
  • Demonstrated high level of computer literacy
  • Must be open to travel to different sites within the New England area for training (accommodation provided if required)


Requirements:

  • Must hold a qualification in one of the following: Occupational Therapy, Nursing, Physiotherapy, Social Work, which provides eligibility or membership of the relevant professional association / provisional or general registration through the Australian Health Practitioner Regulation Agency (AHPRA)
  • Eligibility to drive in NSW including willingness to travel to conduct home visit assessments.
