Position Overview

We are looking for passionate and reliable Community Workers to provide direct support to individuals with disabilities in line with their NDIS goals and personal needs. This role involves supporting participants at home and in the community to build independence, improve daily living skills, and enhance their quality of life.

Key Responsibilities

  • Provide person-centred daily living supports and community access

  • Support participants with personal care, meal preparation, mobility, and household tasks

  • Assist participants to achieve their goals, increase independence, and build capacity

  • Facilitate social and community participation, including transport support where required

  • Support participants with medication prompts and routines (where applicable)

  • Maintain clear and accurate shift notes, progress reports, and documentation

  • Follow behaviour support plans, risk assessments, and individual care plans

  • Support participants in developing life skills, confidence, and connections

  • Ensure safe work practices and uphold participant rights, privacy, and dignity

  • Collaborate with families, support coordinators, and the wider support team Actively reach out to at-risk individuals or communities and engage them in support services, whether that’s through one-on-one conversations, group work, or community events.

  • Outreach and Engagement: Actively reach out to at-risk individuals or communities and engage them in support services, whether that’s through one-on-one conversations, group work, or community events.

  • Case Management: Develop and implement plans for individuals or groups, helping them access services such as housing, healthcare, or employment.

  • Support and Advocacy: Advocacy for the needs of the clients, ensuring they have access to appropriate services and support.

  • Referral Services: Help connect individuals with other professionals or services they may need (e.g., legal aid, addiction treatment, mental health services).

  • Community Development: Work on programs that strengthen community resilience, foster social inclusion, and reduce inequalities within a community.

Skills & Qualifications

Essential:

  • Certificate III or IV in Disability, Community Services, Individual Support, or relevant experience

  • Current NDIS Worker Screening Check / Ochre Card or willingness to obtain

  • First Aid & CPR certification

  • Valid driver’s licence and reliable vehicle (if transport support is required)

  • Good communication, interpersonal, and problem-solving skills

  • Compassionate, patient, and able to work independently and in a team

Desirable:

  • Experience working with people with disabilities, challenging behaviours, or complex needs

  • Mental health or Behaviour Support experience

  • Knowledge of NDIS practice standards and person-centred care

  • Ability to speak a second language or understand cultural diversity
