In your role as the Aged Care Social Worker at Coledale District Hospital you will bring your knowledge and experience in Aged Care to provide a high quality social work service to patients, clients, consumers and their family/carers. The nature of the role includes complex interpersonal work in an effort to build rapport to effect good patient care, a high volume of NCAT and Guardianship processes, and building relationships with external provides when supporting patients, families and carers to navigate the Aged Care system. Your experience will ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.

The Illawarra Hospital Group covers acute and sub-acute clinical settings across Coledale, Bulli, Wollongong, and Shellharbour Hospitals. This position has the possibility to rotate across clinical settings within these hospitals and across sites.

Coledale Hospital is a 38-bed facility specialising in aged care services for the northern Illawarra. The hospital cares for patients transitioning to residential aged care facilities, has a purpose-built unit for patients with a dementia illness awaiting nursing home placement, and is fully accredited by the Australian Council on Healthcare Standards.

Located near the pristine Coledale Beach and surrounded by lush landscape, Coledale Hospital offers a beautiful backdrop to live and work. Fun fact: there's the chance to see whales or dolphins from the hospital grounds during migration season.
