About The Role

This is a unique opportunity to broaden your scope of practice as a Registered Nurse while playing a key role in a service central to connected care pathways across Western NSW. You’ll be at the forefront of improving patient outcomes through coordinated, patient-centred care that bridge services and supports better health outcomes across the region.
Operating Monday to Friday, 8:30am to 5:00pm, you will play a vital role in assessing and supporting individuals as they transition from acute care into community-based and out-of-hospital care programs. A key focus of your work will be improving patient flow and access within the Local Health District, ensuring timely, efficient, and equitable healthcare delivery.
Working closely with a multidisciplinary team, you will facilitate timely, culturally safe, and well-coordinated access to services through expert care coordination and case management. By guiding patients throughout their care journey, you will be instrumental in reducing hospital length of stay, preventing unnecessary and unplanned Emergency Department visits, and promoting safe, effective out-of-hospital care across the region through our Planned Care for Better Health program. Your efforts will improve access, strengthen continuity of care and ensure individuals are connected with the right services and supports.
