About the role:

Are you passionate about supporting individuals through their most challenging moments and helping them to move towards hope and recovery? AnglicareCQ is seeking a compassionate and proactive Support Facilitator - Outreach to join our team and deliver essential support through The Way Back Support Service.

In this role, you will provide assertive, timely psychosocial support to individuals who have experienced a suicide atempt or suicidal crisis. With a strong outreach focus, you'll work across a hybrid model of telehealth, phone-based support and regular face-to-face outreach - inclduing monthly visits to Far West communities. The position centres on proactive and persistent engagement, reducing barriers and ensuring access for people living in remote and very remote locations.

What will a typical day look like?

  • Engage participants facing isolation or access barriers with persistent, flexible outreach.
  • Complete assessments, safety plans, support plans, and facilitate warm referrals.
  • Maintain accurate records, improve service processes, and stay informed on service systems and pathways.

Mandatory Requirements:

  • Certificate IV qualifictions in a mental health or related field.

  • A strong communicator, able to quickly build rapport and influence across a diverse range of people.

Other Requirements:

  • Full COVID-19 Vaccination
  • Ability to obtain a Blue Card (working with children check positive notice or exemption)
  • Ability to obtain a Yellow Card (disability services positive notice or exemption notice)
