About us:
The Transition Care Program (TCP) provides short-term care that seeks to optimise functioning and independence of older people after a hospital stay. TCP is goal-orientated, time-limited and therapy focused. It provides older people with packages of services that include low intensity therapy, such as Physiotherapy and Occupational Therapy as well as Social Work, nursing support or personal care. It seeks to enable older people to return home after a hospital stay rather than enter residential care prematurely.
The program facilitates a continuum of care form older people who have completed their hospital episode, including acute and subacute care (e.g. rehabilitation, geriatric evaluation and management) and who need more time and support to make a decision on their long-term aged care options.
About the role:
The Personal Care Worker works within the Transition Care Program in community settings according to Commonwealth guidelines and the Aged Care Act 1997, supporting the Clinical multi-disciplinary team to provide rehabilitation therapy to frail aged clients post an acute hospital stay. In particular this position provides personal, domestic, shopping and transport to the clients while they are on the TCP program.
To support clients with in-home services to assist them in their rehabilitation journey and remain in their home while on the TCP program. To report any changes to services, or concerns regarding the clients to the Administration Officer who oversees the rostering of the Personal Care Workers. Issues will then be escalated to the client's Case Manager or the Nurse Unit Manager (NUM) of TCP. Ability to work under broad supervision and be able to undertake a diverse range of tasks as an effective member of a team in a high-pressure, high-volume work environment. Demonstrated communication skills, good comprehension, good verbal skills and the ability to deal courteously with people at all levels.
About you:
While not mandatory, a Certificate III in Home Care or Aged Care or equivalent work experience is highly desirable. Highly desirable a Certificate IV in Home Care or Aged Care or equivalent work experience.
What we offer:
Total remuneration value of up to $63,501 p.a. is comprised of:
