25+ hours per week

Community 3SIX5 is seeking an experienced Support Worker to provide community access and daily living supports to NDIS participants across the Gold Coast.

Key Requirements:

• Availability for a minimum of 25 hours per week
• Availability across the week, including weekdays (and weekends if required)
• Demonstrated ability to support participants with challenging behaviours
• High-level spoken and written English for communication and documentation
• NDIS Worker Screening Clearance
• Current First Aid & CPR or willing to obtain
• Driver’s Licence, reliable vehicle and comprehensive insurance

About the Role:

• Community access and social participation
• In-home daily living supports
• Implementation of support and behaviour plans
• Accurate progress notes and incident reporting
• Professional, person-centred support delivery
