About The Opportunity
The Mental Health Triage Clinician role is based at the Citigate Community Mental Health Centre, operating Monday to Friday, 8:30 am - 5:00 pm. This position provides timely, non-acute mental health assessments and care coordination for consumers experiencing moderate to severe mental illness.
In This Role You Will
As part of a multidisciplinary team, you will deliver recovery-oriented, client-centred care that is collaborative, family-inclusive, and trauma-informed. The role involves:
Responding to referrals from the NSW Mental Health Telephone Access Line in a timely manner.Undertaking assessment, planning, implementation, and evaluation of mental health interventions.Coordinate care for consumers, including those on Community Treatment Orders/Forensic Orders where appropriate.Operate in a collaborative care model with consumers, carers, healthcare teams, and external providers to ensure continuity of care.Mentor and support less experienced staff and students in the application of clinical knowledge.

About You
We are looking for someone who has:
Tertiary qualifications in Nursing (Registered Nurse), Social Work, Occupational Therapy, or Psychology with current professional registration.Current driver’s licence.Vaccination Category A / immunisation compliance.Demonstrated ability to work effectively and efficiently in a range of mental health settings.Strong verbal, written, and interpersonal communication skills.Ability to work autonomously and collaboratively as part of a multidisciplinary team. Understanding of recovery models in mental health and the ability to implement them in clinical practice.
