Deliver Occupational Therapy clinical services and case management to eligible clients within our Southport facility

The Role

An opportunity has become available for an Occupational Therapist (Transition Care Program) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

  • Provide accurate assessment, intervention and evaluation of clients with chronic and complex conditions
  • Provide case management services including the coordination of the client's assessment, care planning and discharge planning
  • Provide holistic and client centred care in a highly responsive environment
  • Contribute to the identification, development and implementation of quality and/or clinical research initiatives

About you

To be successful in this position you will require:

  • Tertiary degree in Occupational Therapy
  • AHPRA Registration
  • Aged Care Check (GCH will cover cost)
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Driver Licence
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Temporary part time (up to 6 months with possibility of extension, 64 hrs p.f.)
  • Base salary ranging from $39.86 - $58.59 per hour

Transition Care provides short-term care that seeks to optimise the functioning and independence of older people after a hospital stay. Transition Care is goal-oriented, time-limited and therapy-focused. It provides older people with a package of services that includes case management, low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care to maintain and improve physical and/or cognitive functioning.
